To send a referral, please fax the following information to 416-849-2252:​

Patient demographics

Urgency (Routine, Urgent, Emergent)

Reason for referral, with history & CPP

Any relevant blood-work and/or imaging reports

All referrals are reviewed by the physicians and triaged according to urgency. If you have any concerns about urgent or emergent patients, please call us directly.
​
In order to triage in a timely fashion, please ensure that your referrals are legible and complete.
​
We will contact your patients directly to book their appointment, and we will send you a notification.

WHAT WE TREAT​

Inflammatory arthritis (Rheumatoid Arthritis, Psoriatic Arthritis, Ankylosing Spondylitis)

Connective Tissue Disease ( i.e. Lupus, Myositis, Scleroderma)

Vasculitis

Polymyalgia Rheumatica, Temporal Arteritis

Gout

Cortisone Injections for: rotator cuff tendonitis, knee osteoarthritis, bursitis, and trigger fingers
WHAT WE DO NOT TREAT
Due to the large number of referrals we receive, we are unable to see the following:

Fibromyalgia[1] / Chronic Pain
-
Degenerative Disc Disease
-
Sciatica
-
Patient currently has a treating rheumatologist
-
Positive ANA with no other specific signs or symptoms of SLE or connective tissue disease[2]
