REFERRALS

To send a referral, please fax the following information to 416-849-2252:​

  • Patient demographics

  • Urgency (Routine, Urgent, Emergent)

  • Reason for referral, with history & CPP

  • Any relevant blood-work and/or imaging reports

All referrals are reviewed by the physicians and triaged according to urgency.  If you have any concerns about urgent or emergent patients, please call us directly.

In order to triage in a timely fashion, please ensure that your referrals are legible and complete.

We will contact your patients directly to book their appointment, and we will send you a notification.

WHAT WE TREAT​

  • Inflammatory arthritis (Rheumatoid Arthritis, Psoriatic Arthritis, Ankylosing Spondylitis)

  • Connective Tissue Disease ( i.e. Lupus, Myositis, Scleroderma)

  • Vasculitis

  • Polymyalgia Rheumatica, Temporal Arteritis

  • Gout

  • Cortisone Injections for: rotator cuff tendonitis, knee osteoarthritis, bursitis, and trigger fingers

WHAT WE DO NOT TREAT

 

Due to the large number of referrals we receive, we are unable to see the following:

  • Fibromyalgia[1] / Chronic Pain

  • Degenerative Disc Disease

  • Sciatica

  • Patient currently has a treating rheumatologist

  • Positive ANA with no other specific signs or symptoms of SLE or connective tissue disease[2]
