FIRST-TIME VISIT

​For your first visit, please arrive at least 15 minutes prior to your appointment time.

Please bring the following with you:

​

  • Your OHIP Card

  • A List of Medications you are taking or bring your medication bottles

  • Reading glasses, if necessary, to complete forms

  • Translator, if necessary

  • Shorts and/or T-shirt (for shoulder and knee examinations)

  • All radiographic images (including X-ray, MRI, CT Scan images) – these may come on CD
