For your first visit, please arrive at least 15 minutes prior to your appointment time.

Please bring the following with you:

Your OHIP Card
A List of Medications you are taking or bring your medication bottles
Reading glasses, if necessary, to complete forms
Translator, if necessary
Shorts and/or T-shirt (for shoulder and knee examinations)
All radiographic images (including X-ray, MRI, CT Scan images) – these may come on CD
